Why do this course?

Understanding Doppler assessment in obstetric ultrasound is crucial because it provides vital information on fetal and placental blood flow, helping to evaluate and monitor fetal well-being and placental function. This technique allows clinicians to detect abnormalities such as fetal growth restriction, placental insufficiency, and conditions like preeclampsia by identifying abnormal blood flow patterns in key vessels like the umbilical artery and uterine arteries. Early detection of these issues enables timely and appropriate interventions, improving perinatal outcomes and reducing the risks of complications. Thus, Doppler assessment is a key tool in ensuring a safer and more effective management of high-risk pregnancies.

Course description

A comprehensive understanding of how Doppler ultrasound works is essential for accurate screening, prediction, diagnosis, and management of a wide range of gynecological conditions and pregnancy complications. In this course we will explore; Fetal circulation and the key fetal and maternal vessels that are used within obstetrics.

This overview course aims to explore the fundamentals of Doppler in ultrasound, detailing the precise mechanisms that govern its functionality and ensuring its safe application. We will explore critical aspects such as the selection of vessels to be measured, the optimal timing during gestation for these measurements, and the nuanced interpretation of the obtained results. Doppler also plays an extensive role in cardiac evaluation and anatomical assessment however Doppler investigation into fetal anomalies will be covered in the courses on first and second trimester anatomical evaluation that can be found in other modules of the online Advanced Training Curriculum.

By providing a comprehensive understanding of Doppler ultrasound in obstetrics, this course will equip participants with the knowledge and skills necessary to navigate the complexities of utilizing this diagnostic tool within ultrasound in obstetrics.

Learning objectives

  • Understand principles of fetal circulation.
  • Understand when Doppler measurements of fetal vessel such as the umbilical arteries, the middle cerebral arteries, the uterine arteries and the ductus venosus are indicated and how to interpret and manage the results.
  • Understand when Doppler measurements of maternal vessels such as the uterine arteries are indicated and how to interpret and manage the results.
  • Understand how technical proficiency measuring Doppler parameters leads to enhanced diagnostic sensitivity.

Key questions

  • What are the technical procedures for measuring velocities within the umbilical arteries, the middle cerebral arteries, and the ductus venosus?
  • What are the technical procedures for measuring velocities within the uterine arteries in the first and second trimesters?
  • How does uterine artery Doppler contribute to the screening and prediction of preeclampsia?
  • Is uterine artery Doppler better for predicting fetal growth restriction or preeclampsia?
  • Which clinical scenarios are appropriate for use of CPR?
