The incidence of multiple pregnancy is rising, due mainly to advanced maternal age and resultant widespread use of assisted reproductive techniques. Twin pregnancy is associated with a high risk of perinatal mortality and morbidity, particularly in monochorionic pregnancy, with complications of a shared placenta contributing to this risk. Preterm birth prior is common in up to 60% of multiple pregnancies, resulting in an increased risk of neonatal mortality and long-term morbidity.
In the first trimester, ultrasound enables the recognition of chorionicity, amnionicity, and the number of fetuses and their morphology, Ultrasound also allows for the assessment of the cervix via to identify the risk of premature delivery.
Ultrasound is essential for identifying and monitoring twin pregnancies at greater risk of adverse outcomes, such twin-to-twin transfusion syndrome (TTTS) and fetal growth restriction (FGR), and the evaluation of fetal biometry, anatomy, Doppler velocimetry, and amniotic fluid volume is paramount to establishing fetal wellbeing.
