This month’s VISUOG chapter focuses on cervical ectopic pregnancy. Cervical pregnancy is an uncommon form of ectopic pregnancy where the pregnancy implants into the myometrium below the level of the internal os. It accounts for less than 1% of all ectopic pregnancies, occurring in approximately 1 in 9000 pregnancies.

Ultrasound is the primary diagnostic modality, while magnetic resonance imaging can be used as a second-line investigation if the diagnosis is unclear.

The major risk factors include in vitro fertilisation, endometrial injury caused by pelvic inflammatory disease or postsurgical trauma such as Cesarean section or uterine curettage, history of abortions, intrauterine contraceptive device use, intrauterine adhesions, myomas, and structural uterine anomalies.

Early diagnosis and treatment are the key to a successful outcome. However, accurately diagnosing cervical pregnancy remains a challenge because it is often misdiagnosed as an ongoing abortion.
